You've done your research. You've watched the YouTube videos, scrolled the Reddit threads, and read at least three blog posts claiming keto is basically a thyroid superfood. Then you walked into your doctor's office, mentioned you were thinking about going low-carb, and got a hard pass — maybe even a stern look.

So who's right?

The honest answer is: both sides have a point, and neither is telling you the full story. Let's fix that.

Why Doctors Get Nervous About Keto and Thyroid

Endocrinologists aren't being overcautious for fun. There's legitimate physiology behind their hesitation.

The thyroid gland regulates your metabolism through two key hormones: T4 (thyroxine) and T3 (triiodothyronine). T4 is the inactive storage form; T3 is the active one your cells actually use. Most of the conversion from T4 to T3 happens in the liver and gut — and here's where keto enters the picture.

Several studies have shown that very low-carbohydrate diets can reduce circulating T3 levels. A frequently cited study from the 1970s found that subjects on a zero-carb diet saw significant drops in T3, even when calories were kept the same. More recent research has echoed this, showing that carbohydrate restriction tends to lower active T3 while leaving T4 relatively stable.

For someone with an already-struggling thyroid — say, Hashimoto's thyroiditis or hypothyroidism managed with levothyroxine — even a modest dip in T3 conversion could mean more fatigue, more brain fog, or a medication dosage that suddenly seems off.

"The concern isn't that keto causes thyroid disease," explains one endocrinologist in a clinical practice outside of Chicago. "It's that if someone's thyroid is already working at the margins, reducing T3 conversion can tip them into symptomatic territory that's hard to distinguish from normal keto adaptation."

That's a real and fair concern. But it's not the whole story.

What the Pro-Keto Side Gets Right

Here's what the forums aren't wrong about: lower T3 on a ketogenic diet doesn't automatically mean your thyroid is broken or that you're in danger.

Some researchers argue that reduced T3 during carbohydrate restriction is an adaptive response — the body shifting into a more efficient metabolic mode rather than a pathological one. When you're burning fat for fuel, your cells may simply need less T3 to maintain the same energy output. Think of it like switching from a gas-guzzling truck to a hybrid; you're covering the same ground with less fuel.

A 2017 study in the European Journal of Nutrition found that while ketogenic diets did lower T3, participants didn't show the clinical symptoms typically associated with hypothyroidism — their resting metabolic rates stayed stable, and they didn't feel worse. The researchers concluded the change might represent physiological adaptation rather than dysfunction.

There's also evidence that keto can be genuinely helpful for people with Hashimoto's, an autoimmune thyroid condition. By reducing overall inflammation and potentially lowering autoimmune reactivity, some patients report fewer flares and better quality of life on a low-carb diet. The research here is still early, but it's not nothing.

The Part Nobody Tells You: It Depends on Your Specific Situation

This is where blanket internet advice — in either direction — falls apart. The keto-thyroid relationship isn't one-size-fits-all. Here's what actually matters:

Your baseline thyroid status. If your thyroid function is normal and your labs are solid, a moderate reduction in T3 conversion is unlikely to cause you real problems. If you're already dealing with subclinical hypothyroidism or your TSH is hovering at the high end of normal, you're in a different conversation.

Whether you're on thyroid medication. Levothyroxine (Synthroid) doses are calibrated to your current metabolism. Significant dietary changes — especially ones that affect T3 conversion — can shift how much medication you actually need. This isn't a reason to avoid keto; it's a reason to monitor more closely.

How aggressive your carb restriction is. There's a meaningful difference between eating 100 grams of carbs per day and eating 20. The studies showing the biggest T3 drops tend to involve very strict restriction. A slightly more moderate low-carb approach might give you most of the metabolic benefits with less hormonal disruption.

Your iodine and selenium intake. Both minerals are essential for thyroid hormone production and T4-to-T3 conversion. Keto diets that are heavy on meat and light on seafood and Brazil nuts can inadvertently create micronutrient gaps that stress the thyroid. This is fixable, but you have to know to look for it.

A Practical Testing Protocol if You Want to Try Keto Anyway

If you have a thyroid condition and you're determined to explore keto — or if you're currently on keto and starting to wonder about your thyroid — here's a reasonable monitoring framework that most endocrinologists would support:

Before you start: Get a full thyroid panel, not just TSH. You want TSH, Free T4, Free T3, and if Hashimoto's is a concern, TPO antibodies. This gives you a real baseline to compare against later.

At 6-8 weeks in: Run the same panel again. This is roughly when your body has adapted to ketosis and any dietary-driven hormonal shifts will be visible. If your Free T3 has dropped significantly and you're feeling symptomatic — more fatigued, colder, gaining weight despite eating well — that's a signal worth discussing with your doctor.

Ongoing: Every 3-6 months if you're on thyroid medication. Your prescribing physician needs to know you've changed your diet significantly. Dosage adjustments may be warranted.

Watch your micronutrients: Aim for 150 mcg of iodine daily (easy to get through iodized salt or seafood) and make sure you're hitting selenium through a couple of Brazil nuts or a quality supplement. These are non-negotiable for thyroid support on any diet.

Strategic Adjustments Worth Considering

If labs or symptoms suggest your thyroid isn't loving strict keto, you don't necessarily have to quit. A few targeted tweaks can make a real difference:

Your doctor's caution isn't baseless, and the keto community's optimism isn't entirely wrong. The truth lives somewhere in between, shaped by your individual thyroid health, your lab numbers, your medications, and how strictly you're restricting carbs.

What you shouldn't do is pick a side based on whoever sounds most confident online. Get tested. Talk to your doctor with actual data in hand. And if you decide to move forward with keto, do it with eyes open and regular monitoring — not blind faith in a forum post.
